How to Enroll in Sitike’s DUI First Offender Program without a Court Order:

1. Download Form DMV INF1125
2. Fill out the form, print it and bring it to any DMV office. There is a $5 charge for the Official DMV record.
3. Be Sure that the “Certify the record as true copy of Record” box is checked
4. Be sure that the “Driver License/ID Record” box is checked.
5. Bring printout to your enrollment appointment. If you do not have an appointment scheduled, please call us at 650.589.9305 for an appointment.
